- School physicals
- Weight loss
- Smoking cessation
- Healthy lifestyle counseling
- Depression and anxiety
- Removal of benign skin lesions/warts
- Simple dermatology procedures
- Aspiration and/or injection of joints
- Abscess incision and drainage
- Wound repair and sutures
- Ear wax removal
- STI testing
- The following labs are complimentary once per year with your adult membership:
- CBC (complete blood cell count)
- CMP (chemistry panel)
- TSH (thyroid)
- Fasting lipid (cholesterol levels)
- Urine Analysis
- Vitamin D
- PSA (prostate lab – as indicated)
- Lab work other than the complimentary annual labs will be offered at a discounted price, and these prices will be communicated with you at the time of service.
- X-rays will be offered at a discounted price through partnership with local imaging facilities.
Some procedures have a small fee to cover the cost of supplies, these include:
|Joint Injection (steroid).
|Mole removal/Skin biopsy*
|Wound repair (stitches, staples, glue)
We do not provide vaccinations; we will visit with you about cost-effective vaccination options.
Additional Labs & Imaging
Labs other than the routine annual labs included with membership are provided at “wholesale” pricing without mark-ups, a significant cost savings.

Services not covered:
- OB/pregnancy management
- CDL physicals
- Chronic pain management — We are not accepting patients for the pharmaceutical management of chronic pain. Individuals who use chronic prescription pain medications are encouraged to seek care with a pain specialist.
Because we partner with you to achieve your best possible health, Vibrance Family Health + Care provides these services to help make your health care more convenient and accessible
You’ll be able to call the clinic and your provider during normal clinic hours for routine matters, and you’ll be able to reach your provider after hours for urgent needs.
Email your provider and access your health records through your secure patient portal, Passport.
We offer video (Telehealth) visits and can often use these visits in place of in-person visits in order to make accessing health care more convenient for you.
You can make an in-person office visit during regular clinic hours, 8:30 a.m. to 5 p.m., Monday through Thursday.
If you call or email us with an urgent need before noon on a normal office day (Monday through Friday), we’ll make every reasonable effort to see you the same day. If you call or email us after noon with your urgent request, we’ll try our best to see you the following normal office day – if we can’t see you that same day. We do not accept walk-ins
Appointment Waiting Time
We won’t keep you waiting and waiting when you come in for an appointment. We’ll get you in right away or after a minimal wait. If we foresee a short wait, we’ll get in touch with you to let you know.
Additional visit information
Doctor visits at clinic, during regular hours and communications (email, phone, text) are provided without any additional fees.
We do charge a convenience fee for:
|House calls (available only at provider’s discretion)
|After-hours visits (available only at provider’s discretion)
Non-Medicare family members visiting a patient member from out of town may, for one month, take advantage of the non-medical services described above. A patient member’s visitors will be charged $50 per visit for medical services, plus routine prices for additional lab or radiology services.
When you’re referred to a medical specialist, we’ll coordinate with them to help you receive specialty care. Please understand that membership fees paid under your agreement with Vibrance don’t cover specialists’ fees or fees due to any medical professional other than a Vibrance Family Health provider.
Membership Fee Information
All members must enroll for auto-pay of invoices through credit card, debit card or checking account. Due to our small staff and low overhead business model, we cannot invoice or bill membership or other fees. Auto-pay dates will occur on a monthly basis beginning from the date of registration.
A one-time, non-refundable enrollment fee will be charged to each new billing account.
Additional Service Fees
All services not covered by membership fees (such as labs, meds, procedures, etc.) must be PAID IN-FULL AT THE TIME OF SERVICE unless you’ve made other arrangements with our staff.
If you can’t make it to your appointment, we ask that you cancel at least four hours before the scheduled time. If you don’t show up for an appointment or are excessively late, you will be charged $25 for the missed visit. Exceptions may be granted at our discretion.
Memberships require NO long-term contracts.
You may cancel your membership at any time, for any reason, by providing written notice to Vibrance Family Health + Care.
*Full details regarding membership are included in the Vibrance Family Health + Care patient agreement and can be reviewed when you enroll online or in person. Patient must sign the membership agreement contract in person at the first appointment.
