Upon joining you will pay a monthly membership fee which begins at the time of registration. There is also a one-time, nonrefundable enrollment fee paid at the time of enrollment.
ALL members must enroll for Auto-Pay through automatic bank draft, a pin-less debit card, or a credit card. This is done in order to keep overhead low so that we can pass on cost savings to you.
A one-time, non-refundable enrollment fee will be charged to each new billing account:

Membership fee(s) are started upon enrollment. Membership fees are paid by the calendar month on an ongoing basis until cancellation. Non-Medicare family members visiting a patient member from out of town may, for one month, take advantage of the non-medical services described above. A patient member’s visitors will be charged $50 per visit for medical services, plus routine prices for additional lab or radiology services.
Membership requires no long-term contracts and you may cancel your policy at anytime, by providing 30 days written notice to Vibrance Family Health + Care.
If a member discontinues membership – by choice or lack of payment – the member may rejoin Vibrance Family Health + Care and will be required to pay an “Re-Enrollment” fee of $175.00 in addition to other standard charges to re-establish your membership(s) as active.
All cancellations for scheduled visits must be done a minimum of 4 hours prior to scheduled time. If you do not show for your appointment, or are excessively late for your scheduled time, a $25.00 fee may be charged for the missed visit. Exceptions may be granted at our discretion.
Services + Fees
All services not covered by membership fees (such as labs, meds, procedures, etc.) must be PAID IN-FULL AT THE TIME OF SERVICE unless you’ve made other arrangements with our staff. Members may pay by debit card or credit card, and some charges may be added to the next month’s invoice at our discretion.
Office appointments at the clinic, during regular hours and communications (email, phone, text) are provided without any additional fees.
We do charge a convenience fee for:
- House calls (available only at provider’s discretion) = $100.00
- After-hours visits (available only at provider’s discretion) = $100.00
