Payment Policies

You are here: Home / Members / Payment Policies

Upon joining you will pay a monthly membership fee which begins at the time of registration. There is also a one-time, nonrefundable enrollment fee paid at the time of enrollment.

ALL members must enroll for Auto-Pay through automatic bank draft, a pin-less debit card, or a credit card. This is done in order to keep overhead low so that we can pass on cost savings to you.

Membership Fees

Individual (age 19+)$100.00
Individual + Spouse$180.00
Child (age 0-18)$50.00
Family 5+$300.00

Membership Details

Membership fee(s) are started upon enrollment.  Membership fees are paid by the calendar month on an ongoing basis until cancellation. Payment of membership fees is required to remain an active member and receive service. Prices are subject to change.

Enrollment Fee

A one-time, non-refundable enrollment fee will be charged to each new billing account:

Individual:$60.00
Family (2+ members):$100.00

​ Visitors

If you are a direct primary care member and have a family member visiting from out of town who has an unexpected health care need, they have the option to receive care at the clinic for a $60.00 charge per visit plus routine prices for additional lab or imaging services.

Cancellation

Membership requires no long-term contracts and you may cancel your policy at anytime, by providing 30 days written notice to Vibrance Family Health + Care.

“No-show” charges 

All cancellations for scheduled visits must be done a minimum of 4 hours prior to scheduled time. If you do not show for your appointment, or are excessively late for your scheduled time, a $25.00 fee may be charged for the missed visit. Exceptions may be granted at our discretion.

Services + Fees

Service Fees

All services not covered by membership fees (such as labs, meds, procedures, etc.) must be PAID IN-FULL AT THE TIME OF SERVICE unless you’ve made other arrangements with our staff. Members may pay by debit card or credit card, and some charges may be added to the next month’s invoice at our discretion.

Visits

Office appointments at the clinic, during regular hours and communications (email, phone, text) are provided without any additional fees. 

We do charge a convenience fee for: 

  • House calls (available only at provider’s discretion) = $100.00
  • After-hours visits (available only at provider’s discretion) = $100.00
